24-003: Failed to keep his body-worn camera activated as he initiated a pursuit of the suspect's vehicle. Failed to follow prescribed pursuit policies and procedures. Failed to immediately report the motor vehicle collision (MVC) when he witnessed the suspect vehicle veer off the roadway. Failed to follow prescribed pursuit policies and procedures.
