24-001: Failed to keep his body-worn camera activated during a major motor vehicle collision investigation. Failed to ensure that the appropriate citations were issued for multiple traffic violations. Was untruthful and falsified his submitted police reports. Failed to further investigate and/or recover evidence and failed to scure and process the crime scene.
