25-016: Failed to activate his body-worn camera (BWC) during the incident. Failed to seek immediate medical treatment for the arrestee despite his visible injuries. Failed to document the arrestee's injuries in a Use of Force and Conducted Electrical Weapon report. Failed to photograph the arrestee's inuuries and/or document his failure to do so.
