2021-007: Failed to utilize his blue lights and/or siren to initiate a traffic stopwhen he observed a motorist driving in a hazardous mannerand/or committing traffic infractions. Failed to use his blue lightsand siren and failed to declare a pursuit. Failed to activate hisbody-worn camera, failed to document the omission, and failedto immediately notify his supervisor.
